Demo priceing for truck/trailer

I was woundering how you guys price out your demo disposal? Most guys around here use a dump or dump trailer for remodeling jobs that don't need a big can .Do you charge one price for the truck and dump or do you split it up and charge one set price for the truck and a second for the dump fee?And roughly what is every charging for the truck.

10 yd Dump truck $85.00/hr
Dump fee $.07/lb. if it goes to the land fill.
Stumps and other un prossesed material $40 a 10yd load

I have taken loads to the landfill that have cost the customer over $1,000.00 plus truck time.

If the truck is not moved for over 4 hours then the charge is 1/2 truck rate.
